The American College of Cardiology (ACC) and Peoplechart Corporation jointly developed the CardioSmart Health Tracker for helping you manage your heart conditions. This particular module is aimed at hypertension management and provides you with tracking tools and information to better understand the standards of care as modeled after physician best practices and your physicians’ recommendations for managing your hypertension. The Health Tracker is a part of the ACC patient-education program, CardioSmart.

The CardioSmart Health Tracker (Blood Pressure) follows ACC best-practice guidelines and provides you and your physicians with a way to work together in managing your hypertension. The Health Tracker captures, organizes and presents multiple BP readings, medications, and lifestyle factors in one place. To help explain daily fluctuations, the tool also displays averaged and trended data in context of changes in your individual lifestyle. Using this tool, patients are better able to understand their physicians’ recommendations, the impact of their own behaviors, and share results with their physicians in the next office visit. Our ability to exchange data with Microsoft® HealthVault™ enables you to easily copy the information between our two applications so that your information stays current in both systems.
